Certified Nursing Assistant

Job Location: Lake Havasu City, Arizona

Job Summary: CNA's provide compassionate personal care to patients at the end-of-life in the home setting, with their comfort as your top priority. Under the direction of a nurse case manager, CNA's follow the patient's care plan. Light housekeeping may be included in assigned duties.

Knowledge/Skills:

  1. Hospice philosophy of care.
  2. Provides personal care of patients including bathing, dressing, grooming, feeding, eating, ambulating or toileting.
  3. Maintain a safe environment for the patient including food preparation, cleaning, laundry, changing bed linens and housekeeping.
  4. Maintain timely records of patient care, condition, progress, and problems in order to report and discuss observations with case manager.

Qualifications:

  1. High school diploma or GED certificate.
  2. Current Arizona CNA certification.
  3. Current Adult CPR certification.
  4. Two years professional CNA experience; less considered, if hospice specific.
  5. Availability to be scheduled any day of the week.
  6. Ability to work overtime as requested by supervisor.
  7. Reliable transportation.

Social Worker

Job Location: Lake Havasu City, Arizona

Job Summary: Make home visits to patients and families at the end-of-life, providing psychosocial support. Complete assessments, manage own case load, maintain interdisciplinary care plans.

Knowledge/Skills:

  1. Hospice philosophy of care.
  2. Human behavior and the procedures for assessment of psychosocial issues.
  3. Group behavior and dynamics; societal trends and influences.
  4. Principles and methods of teaching and instruction for individuals and groups.
  5. Computer literate in Windows environment; MS Office.
  6. Develops comprehensive psychosocial assessments and reassessments for patients and families.
  7. Make referrals to appropriate community resources; provide assistance in accessing services (i.e. personal, business, financial, legal, housing, etc.)
  8. Counsel patients and families in individual and group settings to assist them in understanding and coping with end-of-life issues.
  9. Provides short-term crisis intervention when indicated.
  10. Advocate for the best interests of patients and families.

Qualifications: Bachelor's or Master's Degree in Social Work from an institution accredited by the Council on Social Work Education required.
