Transitions, a non-medical no-cost service, may be a reassuring and comforting first step.
Transitions services are administered by social workers and volunteers. We provide:
Assessment of client needs and referrals to appropriate community services and agencies.
Help in understanding and completing advance directives.
Examination of long term care and caregiving options.
Guidance in discussing health concerns and feelings related to life-limiting illness.
Trained volunteers are available to provide friendly visits, help with meals or provide a break for family members.
Transitions is a non-medical service offered at no charge or obligation.
