Transitions

When living with a chronic or serious illness, sometimes it’s not clear where to turn or what to do. The Hospice of Havasu Transitions program may be the best choice for you or a loved one at this time to help answer questions and address concerns. The Transitions team has knowledge about local resources and agencies that help people stay independent and maintain their quality of life. There is no charge to participate in the Transitions program.

The Transitions team and trained volunteers can make home visits or phone calls to check in as needed.

How Transitions can help:

  • Examination of your long term care and caregiving options.
  • Discuss your options for living with a chronic or terminal illness.
  • Assist you in completing healthcare powers of attorney.
  • Refer you to community resources that can help with practical needs, emotional support, and financial questions.
  • Provide guidance and support to caregivers.
